13. Enhancing vestibular compensation through optokinetic stimulation

Summary

Vestibular compensation is a complex phenomenon that is only now beginning to be understood. In many cases, we can only rely on the patient’s clinical progress or on the patient’s own subjective sense of improvement.

Although electrophysiological studies of the oculovestibular reflex (OVR) have shown a resumption of activity following vestibular deafferentation, and there is a certain correlation with the patient’s clinical course, these studies are not sufficient to explain the compensation process in its entirety.

In addition to the existing adaptive changes in the ROV, there are others related to the use of other sensory or motor inputs and the reinterpretation of movement strategies. Regardless of these adaptive changes, cognitive and psychological factors are no less important, and may well be responsible for the considerable variability observed in the clinical course of patients with similar conditions.

In addition to the information obtained from the ROV of the horizontal semicircular canal, thanks to caloric and rotational stimulation tests, dynamic posturography provides us with functional information on the vestibulospinal system and its progression over time.

The aim of this study is to examine two groups of patients with peripheral vestibular disorders – those who are compensated and those who are decompensated – and to assess the differences in the parameters studied.
